Can this plan be combined with dental insurance?
No. This is an in-house membership plan designed for patients who do not have dental insurance or choose not to use existing coverage. It cannot be combined with a dental insurance plan.
What services are eligible for discounts?
Plan discounts apply to a range of eligible dental services, including treatments such as fillings, extractions, crowns, bridges and implant restorations. Some exclusions apply, so speak with our team or review the membership plan document for complete details.
Are plan benefits transferable to family members?
Membership fees are non-refundable, and benefits cannot be transferred between family members. Each person must enroll in an individual membership plan.
When should I pay plan fees?
Membership fees are due on the day of enrollment. Plan fees may be paid using cash, check or credit or debit card. Credit card payments incur a 3.5% surcharge.
When can I start using my membership benefits?
Your membership becomes effective immediately upon receipt of payment. The membership period lasts 12 months from your enrollment date, and benefits must be used during that membership period.
Can I still receive discounts if I discontinue my plan?
To receive membership discounts, you must remain an active plan participant for the full duration of your recommended treatment. The membership plan cannot be used for dental treatment associated with injury claims involving external medical, automobile, disability or workers’ compensation insurance.
How do I receive discounted rates for my procedures?
Once you are enrolled, our team will apply available membership discounts to eligible treatment according to the plan terms. No separate insurance paperwork is required.
Is there a waiting period before I can access benefits and services?
No. Your membership benefits are available immediately after enrollment and payment.
How can I cancel my membership?
The membership plan is designed as a 12-month commitment and cannot be canceled before the annual term is complete. If you do not wish to continue after your membership year ends, contact our team to discontinue the plan.
